Social Worker

• Collaborate with healthcare providers, educators, legal professionals, and community agencies
  

• Conduct home visits and community outreach when required
  

• Monitor client progress and adjust care plans as needed
  

• Ensure compliance with ethical standards, policies, and relevant laws
  

Requirements

• Knowledge of social work ethics, safeguarding, and community resources
  

• Strong communication, empathy, and case management skills
  

• Ability to maintain accurate, confidential records
  

• Willingness to conduct fieldwork/home visits
  

• Background and reference checks as required
