Social Worker

The Rivers Grosse Pointe is seeking a candidate to join the team as a PART-TIME Social Services Tech. As a member of the interdisciplinary team, the Social Services Tech provides services to respond to the emotional needs of residents and their families, as well as assessment, documentation, and reporting responsibilities.

In part, the responsibilities include:

• Work with the Social Service Team, starting with the Admission Process, meet with the resident and family, assist in completing admissions paperwork and conduct evaluations and assessments, including identification of resident needs.
• Provide/arrange for social work services as indicated by resident/family needs, transfers, and changes in care as requested
• Maintain a current list of community resources for effective and efficient discharge planning.
• As a team, implementing interventions that achieve treatment goals, address resident needs, and link social supports, physical care, and physical environment to enhance quality of life.
• Assist the team to complete the MDS process in a timely manner and maintain/update care plans as needed including assist to coordinate family meetings and facilitate such meetings to ensure residents and families are kept informed of condition changes and interventions.
