Job Description
Are you passionate about helping young people overcome barriers and reach their full potential? We are seeking a dedicated and relationship-focused Student Success Coordinator to join our Guided Pathways to Success (GPS) Mentoring Program.
The GPS Mentoring Program supports students in grades 7–12 who may be experiencing academic, behavioral, attendance, or social challenges. Using the evidence-based Check & Connect mentoring model, Student Success Coordinators build meaningful, long-term relationships with students while partnering with families, school staff, and community resources to improve school engagement, attendance, academic success, and graduation outcomes.
This position serves as a mentor, advocate, and case manager, working directly with students through individual and small-group mentoring sessions, progress monitoring, and coordinated support services.
Key Responsibilities
Program Development & Implementation
- Develop supportive mentor-mentee relationships with referred students to reduce truancy, improve academic performance, and support compliance with court expectations when applicable.
- Meet regularly with students through one-on-one and small-group mentoring sessions focused on goal setting, leadership development, character building, and life skills.
- Monitor student attendance, behavior, grades, and overall engagement.
- Create consistent schedules within assigned schools to provide mentoring, guidance, and ongoing academic support.
- Manage student intake, case management activities, and program documentation.
- Connect families to Family Outreach Support Services and other community resources when appropriate.
- Ensure mentoring spaces are welcoming, engaging, and provide student recognition opportunities.
- Monitor monthly program expenses and maintain budget awareness.
School Partnership & Collaboration
- Serve as the primary GPS contact for assigned schools.
- Build strong relationships with school administrators, counselors, teachers, and student services personnel.
- Participate in student progress meetings and collaborate on intervention strategies.
- Communicate regularly with school teams regarding student successes, challenges, and program outcomes.
- Work closely with the Assistant Director of GPS and school partners to establish and achieve annual program goals.
Program Evaluation & Reporting
- Monitor and evaluate program effectiveness against established goals and performance outcomes.
- Track participation, attendance, engagement, and student progress data.
- Collect and maintain outcome data to meet program and reporting requirements.
- Prepare accurate and timely reports and case notes.
- Ensure mentoring activities align with evidence-based youth development practices.
- Maintain a safe, positive, and high-quality environment for all participants.
