Do you enjoy building meaningful relationships, helping others reach their goals, and making a difference in someone's everyday life? As a Supported Community Living Case Manager at Latham Centers, you'll have the opportunity to do just that.

You'll work one-on-one with individuals living independently in the community, helping them navigate everyday life while encouraging independence, confidence, and personal choice. From planning appointments and managing finances to grocery shopping, community activities, and everything in between.

You'll become an important and trusted part of the individuals' lives while working alongside a supportive team of coworkers, clinical professionals, and supervisors.

Compensation: $20.96–$22.65 per hour, based on experience
Location: Yarmouth Port, MA
Schedule: Monday–Friday, 8:00am-3:00pm

A Day in the Life as an Independent Living Case Manager
Your day might look a little different every day—and that's part of what makes this role so rewarding!
  • Start your morning by checking in with the individuals you support and reviewing the day's plans.
  • Plan and coordinate appointments, vocational activities, social outings, and other important commitments.
  • Get out into the community for grocery shopping, errands, appointments, recreational activities, or other activities that promote independence.
  • Help build independence by supporting individuals with budgeting, financial records, and money-management skills.
  • Support a successful home life by helping with meal planning, food shopping, daily living skills, and maintaining a safe and healthy environment.
  • Build relationships by getting to know each individual—their interests, goals, preferences, and what matters most to them.
  • End your day knowing that the support you provided helped someone have a more successful, independent day.

What We're Looking For
  • High School Diploma or GED required
  • Bachelor's degree in Human Services or a related field preferred
  • Valid driver's license
  • Compassionate, dependable, and patient
  • Strong communication and organizational skills
  • Comfortable working independently while collaborating with a team
  • Ability to physically intervene in instances of extreme behavior
What You'll Do
  • Support individuals with activities of daily living, personal needs, and community involvement
  • Support individuals in developing and maintaining independent living skills
  • Oversee financial records and money-management plans
  • Assist with meal planning, preparation, and grocery shopping
  • Plan and coordinate healthcare, vocational, and social activities
  • Complete monthly financial packets according to established procedures and timelines
  • Provide case management support to maintain a safe and healthy home environment
  • Coordinate transportation to community resources, medical appointments, job interviews, and other activities
  • Provide non-violent crisis intervention and support as needed
