What You'll Do

As a Targeted Case Manager, you'll work directly with children experiencing serious emotional or behavioral challenges and their families to coordinate services that make a lasting difference.

Your responsibilities include:

• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate behavioral health, educational, medical, and community services.
• Connect families with resources and advocate for needed supports.
• Monitor progress and adjust service plans to ensure goals are being met.
• Participate in treatment team meetings and Medicaid staffings.
• Complete accurate documentation in accordance with Florida Medicaid requirements.
• Provide crisis support and help families navigate challenging situations.
• Build collaborative relationships with schools, providers, and community partners.
• Assist with discharge planning and continuity of care.
• Transport clients to appointments when necessary using a SAYS vehicle.
