SUMMARY
The Direct Support Professional (DSP) provides person-centered assistance to adults with developmental disabilities in small residential homes. The DSP supports each individual’s health, safety, dignity, rights, choices, independence, and participation in home and community life. ABOUT US
Community Homes, Inc. has been supporting adults with developmental disabilities since 1983. We provide safe, structured, and home‑like environments where residents can grow, make choices, and live meaningful, connected lives. Our team plays a vital role in helping individuals build independence and enjoy a fulfilling daily routine.
RESPONSIBILITIES
- Follow each individual’s person-centered plan, support documented goals, offer meaningful choices, and encourage the greatest level of independence reasonably possible.
- Provide active supervision and maintain a safe environment for individuals served. Follow emergency procedures, behavior-support plans, infection-control practices, and all applicable agency policies; promptly report hazards, incidents, injuries, suspected abuse or neglect, and changes in health or behavior.
- Assist with all areas of daily living according to assessed needs, including bathing, grooming, dressing, toileting, mobility, transfers, eating, meal planning and preparation, housekeeping, laundry, shopping, budgeting, scheduling, and other household routines.
- Engage individuals respectfully and patiently; teach, prompt, model, and reinforce skills rather than completing tasks for the individual when participation is safe and appropriate.
- Assist with or administer medications only as trained, authorized, and documented. Support appointments, follow health-care instructions within the scope of the role, and complete required records accurately and on time.
- Support transportation, appointments, employment or day-program activities, recreation, relationships, and community activities selected by the individual.
- Help maintain a clean, sanitary, secure, and welcoming home while respecting that the workplace is the residence of the individuals served.
- Communicate professionally with individuals, guardians, families, coworkers, supervisors, and service providers. Complete shift notes, medication records, incident reports, mileage logs, and other required documentation accurately, objectively, and confidentially.
- Attend required training and meetings, follow assigned schedules and staffing procedures, maintain confidentiality, protect individual rights, and comply with applicable laws, regulations, licensing requirements, plans of service, and Community Homes, Inc. policies.
