Temp Patient Care Coordinator - Virtual Assistant (Healthcare) #HZO

Location: West
Working Hours: 5 days office hours

Job Scope:

• Conduct patient outreach and enrolment, ensuring eligible patients are accurately registered.
• Monitor patient responses via chatbot dashboards and follow up with non-responsive patients.
• Coordinate appointments, referrals, handovers and communication between patients and clinical teams.
• Maintain accurate patient databases, interaction records and documentation across IT systems.
• Analyse enrolment data, response rates and patient outcomes, prepare reports, and support audits and process improvements.

Job Requirements:

• Min. Diploma in Health Sciences or any relevant field.
• Customer service or healthcare experience will be an added advantage.
