POSITION OVERVIEW:
This position will report to the Community Health Services Director/Clinic Administrator. The Promotor(a) will work collaborative with the Mobile Health and La Familia Community Health Center team members, including Chief Medical Officer, Registered Nurse, LNV/Care Coordinator, Front Desk staff, Medical Assistant, Eligibility Coordinator and La Familia Behavioral Health Dept staff and Community partners as appropriate.
The Promotor(a) de Salud supports community outreach, health education, patient navigation, and engagement activities for the Mobile Health Program and Community Health Center. Working as a trusted community liaison, the Promotor(a) helps individuals and families access preventive health services, health insurance, and community resources while promoting continuity of care.

This is a temporary position (until December 2026) – weekend availability is required.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
Community Outreach
- Conduct outreach within neighborhoods, schools, community organizations, and other community settings in Alameda County.
- Promote mobile health clinics and Community Health Center services.
- Recruit participants for health education and screening events.
- Build trusting relationships with community members.
- Leverage existing and form new partnerships to represent cross-sections of participants throughout the county.
- Develop relevant, culturally appropriate, semi-structured, and qualitative questions with focus on participant experience accessing or receiving county services.
Health Education
- Provide culturally appropriate health education on preventive care, immunizations, chronic disease prevention, and healthy lifestyles.
- Distribute educational materials and promote available health services.
Patient Navigation
- Assist individuals with scheduling appointments and accessing healthcare services.
- Support referrals to medical, dental, behavioral health, and community resources.
- Conduct reminder and follow-up calls.
- Help reduce barriers to care, including transportation and language needs.
- Provide timely, thoughtful, evidenced-based, trauma-informed, culturally responsive responses to participants and all partners.
- Provide social service and benefit enrollment referrals (including housing, Social Security Disability benefits, CalFresh, etc.) and follow up to ensure referral utilization, with an emphasis on reducing stressors and barriers to regular, reliable care.
Mobile Health Support
- Assist with mobile clinic setup, patient flow, registration support, and community engagement.
- Collaborate with nurses, providers, Registration staff, Eligibility Specialists, and CHWs to ensure efficient operations.
Documentation
- Document outreach activities, referrals, and follow-up services.
- Submit required documentation within the 48 hour standard timeframe
- Maintain confidentiality and comply with HIPAA requirements.
- Participate in team meetings and required training.
Other
- Attend training and participate in events relevant to key responsibilities.
- Track and regularly report on key metrics for the responsible function areas.
