Cornerstone – ACT Team Therapist

Cornerstone empowers people living with mental health and co-occurring substance use disorders to live, work, and integrate successfully into the community. Responsibilities of the Therapist

• Provide services and supports, such as individual and group therapy, that lessen or eliminate the debilitating symptoms of mental illness and minimize or prevent acute episodes of the illness
• Provide general case management services during meetings as required. For example: filling out paperwork, assisting in scheduling appointments and other general community support services
• Implement theory-based interventions to assist clients in overcoming functional impairments and maximize their individual potential
• Assists in managing emergency and crisis situations both on-site and in the community, through the use of risk screening and assessments and collaboration with other team members and community providers
