Therapy Aide/ Assistant (Jurong West & West Coast)

Job purpose

To support therapists in delivering safe and effective rehabilitation services by assisting in therapy implementation, client monitoring, documentation, and operational coordination for Community Rehabilitation (CR).

  • Mon to Fri, 8.30am to 6pm
  • Location: Jurong West and West Coast centre
  • Permanent role with benefits

Duties and responsibilities

1. Rehabilitation Support

  • Assist in implementing therapy interventions as prescribed by Physiotherapists (PT) and Occupational Therapists (OT).
  • Support clients during sessions to ensure safety, engagement, and adherence to care plans.

2. Client Monitoring & Communication

  • Monitor clients’ condition and promptly escalate any changes or concerns to the therapist.
  • Maintain effective communication with the multidisciplinary team to support continuity of care.

3. Documentation

  • Record therapy progress notes, attendance, and client updates accurately and in a timely manner.
  • Ensure documentation complies with organisational standards and requirements.

4. Scheduling & Coordination

  • Assist in scheduling CR/HT sessions and coordinating appointments to optimise service delivery.

5. Care Plan & Data Support

  • Support monitoring of Individual Care Plans (ICP) and highlight any deviations.
  • Assist in data collection and reporting for service tracking and audits.

6. Equipment & Inventory Management

  • Maintain cleanliness and safety of rehabilitation areas and equipment in line with infection control standards.
  • Conduct routine equipment checks and report faults.
  • Manage monthly inventory and ensure adequate stock levels.

7. Quality Improvement & Training

  • Participate in in-service training, case discussions, and rehabilitation journal sharing.
  • Support and contribute to Quality Improvement (QI) initiatives.

Requirements

  • Graduated with PT/OT background
  • Relevant experience in CR setting
  • Good communication and interpersonal skills
  • Ability to work in a multidisciplinary team
  • Basic administrative and documentation skills
  • Prior experience in rehabilitation or community care (preferred)
