Thriving Texas Families Program Manager

Position Summary

The Thriving Texas Families Program Manager will report to the Social Services Director, this position will be responsible for oversight and general management of the Thriving Texas Families program. The purpose of this program is to operate a support network statewide that provides community outreach, consultation and care coordination for women with an unexpected pregnancy, which includes prenatal, perinatal and postnatal services. The goal of the program is to improve healthy pregnancies and deliveries; child health and development; and improve family dynamics. The Program Manager will provide oversite and guidance with direct care staff to ensure outreach and implementation of core services are completed to program design standards.

This position manages ancillary staff, organizes schedules, coordinate delivery of program core services, submit monthly reporting, participate in monthly contract calls, and provides leadership, training, ordering program supplies, attending community stakeholder events, and observation of staff conducting services. Core services of the program to the individuals who are eligible for services. This will include pregnant mothers, spouses, legal guardians, or foster parents who are pregnant or has a child between the age of 0-3. Individuals identified in need of crisis support, referrals, case management, or mentorship.

Minimum Requirements

  • Position will represent the program at relevant meetings throughout the community, and be familiar with community resources in order to conduct referrals and meet the needs of the targeted groups.
  • Provide direct supervision to ancillary staff working in the program. This includes schedules, team meetings, assigning caseloads, outreach sites, mileage, weekly schedules, and adherence to policies and procedures.
  • Responsible for maintaining program supplies with emphasis on office supplies, program supplies for participants, submission of approvals, and coordination of picking up supplies.
  • Assisting the Social Services Director in submission of monthly programmatic reports. Includes combining all program activities conducted by staff. Reports must be in required program templates to meet required deadlines. Required reporting will include all demographic data, referrals, rates of participation, and follow ups to support annual goals of contract and monthly performance measures.
  • Monitoring and observations of all staff and subcontractors to support quality assurance program performance to address any training needs or corrective action plans and resolutions to staff.
  • Responsible for conducting monthly CQI activities for CPI committee and to review effectiveness of the programming and outcome objectives.
  • Be able to facilitate all program activities including community outreach, recruitment, intake, referrals, client cases, individual group sessions, providing basic needs support, and follow up according to the established policies and procedures to meet required outcome measures.
  • Provides annual performance evaluation reviews for all ancillary staff.
  • Routinely review approve budget and spending requirements.
  • Serve as a Community liaison with community leaders and community coalitions.
  • Ensure all outreach materials are approved, organized, updated, and culturally appropriate.
  • Will report directly to the Social Services Director to assess program needs, participant needs and staff needs to coordinate needed support, training, referrals, etc.
  • Will work closely with the patients in an effort to keep them engaged in the medical system during pregnancy and facilitate referral to social services and basic life needs.
  • Position will communicate with staff any concerns or issues needing immediate attention.
  • Assists Social Services Director in developing new program components, and other related duties.
  • Position requires a flexible schedule and may require working irregular hours.
  • Carries out other duties as assigned.

Physical Requirements

Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment.


The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.


The occupant may, at times, lift and/or move objects up to 30 pounds. The noise level in the work environment is usually moderate.
