Transitional Counselor

Are you passionate about helping others build independence and thrive in their community? BHcare is seeking a Transitional Counselor to provide guidance, support, and skill-building for individuals in our Transitional Supervised Housing Program. This role is essential in empowering residents to reach their personal goals, develop life skills, and enhance their overall quality of life.

Schedule (40 Hours/Week):
• Saturday & Sunday: 8:00 am – 4:00 pm
• Monday, Tuesday & Wednesday: 4:00 pm – 12:00 am

What You’ll Do:
• Assist in the daily operations of the program while ensuring a safe and supportive environment.
• Help clients create and follow individualized recovery plans, reviewing and updating as needed.
• Support residents in developing independent living skills such as hygiene, cooking, cleaning, budgeting, and health management.
• Provide counseling, emotional support, and rehab interventions, including facilitating groups.
• Assist clients with scheduling and attending personal appointments; provide transportation as needed.
• Collaborate with treatment teams, natural supports, and community resources to ensure comprehensive care.
• Conduct crisis assessments and interventions, coordinating with clinical or emergency services when necessary.
• Maintain accurate documentation of client progress, services, and recovery goals.
• Enter daily service activities into the database and ensure data accuracy.
• Stay awake, alert, and responsive during all shifts, prioritizing resident safety and well-being.

What We’re Looking For:
• Compassionate, reliable, and motivated to support individuals in recovery.
• Strong communication, organizational, and problem-solving skills.
• Ability to remain calm and supportive during crisis situations.
• Experience with individuals who have co-occurring disorders is a plus.
• Must have a valid driver’s license and willingness to use own vehicle for company business.
