Position Overview

Under the supervision of the Title VI Program Coordinator, the Van Driver/Minor Care Giver delivers meals to the homebound clientele and provides transportation to elders to the nutritional site. Assists with the coordination of activities and services for program clientele and caregivers in order to allow and enhance the highest possible degree of independent living in the community. Evening or weekend hours may be needed.

Essential Functions

  • Communicates and interacts regularly with clients and their caregivers/family developing and maintaining a positive, supportive rapport.
  • Perform light housekeeping for elderly that may require light lifting.
  • Assists with the elder activities and schedules/provides rides accordingly.
  • Assists in securing transportation to and from meal site, other activities, and appointments if no caregiver exists/CHR is unable to do so.
  • Completes client program records, documentation, and reports in accordance with the Title VI Policy and Procedures as required for the American on Aging funding agency.
  • To understand and comply with the statutory and legal requirements such as HACCP, Health & Safety, COSHH, Environmental Health, to maintain a safe and healthy environment throughout the meal site.
  • Receives and transmits messages between other personnel, clients, families, other agencies, and the general public.
  • Drives van for daily meal deliveries and transporting elders to meal site. Check on the well-being of elders during meal delivery.
  • Makes sure the program van is clean and neat as well as upkeeps vehicle maintenance; will be responsible for submitting monthly Fleet Report and making sure GSA payments are submitted.
  • Cleans all areas of the program dining room with the correct cleaning products following instruction and guidance and ensure that the Title VI Senior Program is cleaned to its highest standards.
  • Maintains morning duties such as making tea/coffee, setting up the dining room tables, and getting containers ready for the homebound and Pawnee Indian Health Center meal delivery.
  • Interacts with elder participants in a warm, professional manner, maintaining positive relationships with the participants.
  • Responsible for ensuring emergency equipment required by the state (i.e. fire extinguisher, first-aid kit) are available and in good working order.
  • Serves as back-up for Cook when needed;
  • Helps with the serving of meals, when necessary, be understanding of any specific food requirements and ensure everyone is treated with respect and dignity.
  • Responsible for delivery on Food Bank evenings.
  • Maintains confidentiality of all privileged information.
  • Contributes to a team effort and accomplishes related results as required.
  • Performs other duties as assigned by the program coordinator and within the scope of the position.
