VMAP Licensed Mental Health Professional, Department of Pediatrics
Description
HealthHaven includes multiple programs designed to increase capacity of primary care clinicians (PCCs) and other prescribing clinicians to provide care for common mental health and behavioral health concerns through training, consultation, and resource support. These programs include VMAP, VMAP for Moms+, and APAL. The HealthHaven call line provides consultation and resource support through a hub model. Each regional or specialty hub is staffed by at least one FTE of a consulting physician (CP), a licensed mental health professional (LMHP), and at least one care navigator. The care navigator helps PCCs and their patients identify appropriate local mental health resources and supports.
The VMAP LMHP provides oversight, coordination and management of their respective hub and serves as the initial point of contact for incoming calls to the HealthHaven line for patients ages 21 and under. The LMHP leads development and implementation of an outreach plan, conducting both in-person and virtual engagement with primary care practices to increase program awareness and utilization. LMHPs receive and screen incoming calls from PCCs, provide case conceptualization and diagnosis support, and work alongside consulting physicians (CPs) on consultations. The LMHP also supervises the hub’s care navigators. Care navigators provide direct patient/family support for individuals identified with behavioral health diagnoses who need additional services outside of primary care, including referral to and engagement with external services. The LMHP position works closely with the care navigator to ensure prescribers and patients are directed towards the right referrals and resources based on the case, the consultation, and the patient’s specific needs.
