Affinity Care MN is hiring a mission-driven Waiver Case Manager. We support people throughout the community, whether they live in their own homes or in our residential settings. Our mission is to improve quality of life by empowering people with knowledge, resources, and skills, and by treating everyone with dignity and respect.
The Waiver Case Manager delivers person-centered case management to individuals living with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions. These individuals qualify for services through county-administered waiver programs, including Community Access for Disability Inclusion (CADI), Developmental Disabilities (DD), Alternative Care (AC), Elderly Waiver (EW), and Brain Injury (BI).
Job Responsibilities:
- Carry and maintain a caseload of individuals diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions across the assigned county.
- Meet with clients at least monthly and twice per year as required by waiver-specific rules.
- Manage client intake and discharge within established parameters and timelines.
- Work alongside other professionals, including physicians, mental health providers, and care teams, to build and update each client's plan.
- Confirm that financial authorizations and provider contracts are in place.
- Document services in each individual's record so information stays accessible, outcomes can be measured, and record-keeping standards are met.
- Connect individuals with community and housing resources that fit their needs and preferences.
- Deliver crisis assessment and intervention so individuals stay safe in their home and community.
