The YouthBuild Case Manager provides comprehensive, participant-centered case management and supportive services to YouthBuild participants ages 16-24. The Case Manager works closely with participants to identify barriers, develop individualized service plans, coordinate supportive services, and support educational, employment, and personal development goals throughout program participation.
Participant Case Management
- Conduct comprehensive intake assessments to evaluate participants educational, employment, housing, transportation, financial, mental health, and supportive service needs.
- Develop Individual Service Plans with measurable goals and complete the job readiness portfolio.
- Maintain ongoing case management to identify barriers to participation and coordinate appropriate interventions and referrals.
- Support participants in achieving educational goals, including GED completion, high school diploma attainment, industry-recognized credentials, and post-secondary enrollment.
- Monitor participant attendance, engagement, academic progress, credential attainment, and employment readiness.
- Assist participants with career exploration, career planning, resume development, interview preparation, and employment readiness.
- Encourage leadership development, civic engagement, and life skills growth consistent with the YouthBuild model.
- Assist participants in transitioning to employment, apprenticeships, advanced training, military service, or higher education, in partnership with Employment and Career Pathways team members.
Participant Engagement
- Facilitate workshops related to job readiness, life skills, financial literacy, conflict resolution, goal setting, wellness, and leadership development.
- Assist participants in developing problem-solving, decision-making, communication, and self- advocacy skills.
- Participate in outreach and recruitment efforts as needed.
Documentation and Grant Compliance
- Maintain complete, accurate, and timely participant case files in accordance with organization/grant requirements.
- Enter participant information into required information systems and databases.
- Track participant outcomes and supportive service utilization.
Collaboration
- Collaborate with YouthBuild instructors, career coaches, construction staff, leadership, and external partners to coordinate participant success.
- Participate in multidisciplinary team meetings, staffing discussions, and case reviews.
