Youth Mental Health Clinical Evaluator

The Henry & Rilla White Youth Foundation, a non-profit human services organization, is seeking a masters-level clinician to work in the Department of Juvenile Justice referred youth in Circuit 4, serving Clay, Duval, and Nassau counties.

The position is for a contract employee to complete intake and a diagnostic mental health evaluation for youth ages 13-19 currently on probation from the Department of Juvenile Justice.

Intake and Evaluation Process

Referrals will be issued by Foundation staff to clinician

Schedule intake session to complete consent paperwork, screening assessment, and Bio-Psychosocial

Complete Evaluation Report (Template provided) and submit to Clinical Director within 10 days of receipt of referral.

Compensation: Fee for Service arrangement and will be negotiated based off experience.

QUALIFICATIONS

Minimum Qualifications include: Master's Degree from an accredited college or University in counseling, social work, psychology, mental health counseling, marriage and family therapy or related field. Training and experience in Cognitive-Behavioral therapy, structural family therapy, and other behaviorally-based treatments are preferred (but not required).

Other Requirements: Clinician will be need to have the ability to print and scan required signature pages for intake forms and evaluation. Foundation will provide a secure gateway to upload those forms. Compensation for this use will be factored into the fee for service agreement.
