Youth Transitions Support Worker

POSITION OBJECTIVE

The Youth Transitions Support Worker provides vulnerable youth with the community supports
and life skill development they need for educational attainment and future success. Program
delivery includes, but is not limited to, the development of youth-centered plans, facilitation of
workshops, and one-on-one support. The Youth Transitions Support Worker uses an outreach
model to meet youth within the community, at schools, local YMCA facilities or another
appropriate site. The SW is responsible for the effective planning, implementation, and
evaluation of program services. The SW carries out their duties in a professional manner and in
accordance with all YMCA policies and procedures, standards of practice, health and safety
standards, child protection standards and contractual, legal, and regulatory requirements

MAIN RESPONSIBILITIES

  • Adherence to the YMCA Child Safeguarding Policies and Procedures ensuring a

safe environment for children, young people/vulnerable adults; identifying
children, young people/vulnerable adults who are suffering, or likely to suffer,
significant harm; and taking appropriate action to see that such children, young
people/vulnerable adults are kept safe at the YMCA.

  • Adherence to the YMCA Occupational Health & Safety Policy and accountable to

work safely at all times, to identify and report hazards, to take whatever
measures necessary and reasonable in the circumstances, to protect and
promote health and safety.

  • Serve as a role model to members, volunteers, staff, and stakeholders by

demonstrating our YMCA values.

  • Understand, promote, participate and solicit others to participate in YMCA

philanthropic, recognition, and awareness events and activities in the
organization.

  • Ensure Y Way Service Standards and Shine On Branding guidelines are followed

and exceeded.

  • Follow a Clean Culture and conduct spot and light duty cleaning as needed,

complete cleaning (daily, weekly, monthly, and annual) checks, and ensure any
other cleaning issues are addressed immediately.

  • Ensures a safe, comfortable and welcoming environment for participants. · Collects information to confirm eligibility and completes participant intake and/or assessment to inform ongoing participant supports to reach participant and funder outcomes. · Designs, implements, and evaluates youth-centered individual action plans, in-person services and workshops as per guidelines and funder mandates. · Performs case management as per guidelines and funder mandates. · Consults with social workers, school administration, other professionals and family/natural supports when appropriate. · Maintains knowledge of relevant services and community resources. · Engages with schools, community agencies, and other organizations to maximize services and resources to participants. · Inputs data accurately and timely into program designated database systems · Work within defined program budgets · Maintain accurate participant files in accordance with YMCA guidelines · Collect demographic, output and outcome data for evaluation purposes and to inform and contribute to the development of YMCA reports.

QUALIFICATIONS

  • Post secondary degree/diploma in Child and Youth Care Counselling, Education, Social Work,

Recreation Management, or related field.

  • 2 years of experience working directly with vulnerable youth
  • 2 years of experience in client intake and assessment, case management and workshop

facilitation

  • Experience working with diverse populations
  • Experience within a team environment
  • Standard First Aid & CPR-C and able to obtain a Police Information Check and Vulnerable sector • Non-Violent Crisis Intervention, Motivational Interviewing, Suicide Intervention Certification preferred • Valid driver’s license and access to a vehicle, in communities where applicable • Working knowledge of issues/practices related to the social determents of health, youth development, individual assessment, case management, group work, group facilitation, program planning and outcome evaluation • Working knowledge in culturally sensitive and trauma-informed practice, and strength-based approach • Excellent organizational and time management skills • Excellent communication, interpersonal, and team skills •Working knowledge computer skills (Windows, Microsoft Teams, Office and Outlook)

REQUIREMENT
This position shall not, for the duration of their employment, maintain any interests
directly or indirectly, as an employee, or act in any other capacity, for any other
organization that conducts business which may be perceived as similar to, or in
competition with, The YMCA of Greater Moncton’s business, without obtaining prior
permission from the YMCA Department VP or Director. Due to YMCA privacy and
ethical business practices, the employee in this position agree to refrain from contacting
YMCA clients and members for business purposes after employment termination.
