About
Health Choice Network (HCN) is one of the first successful health-center controlled networks in the nation, established in 1994 when a small group of community health centers recognized they were stronger together. By forming a network, they gained the expertise and advantages of a large health system while staying independent and rooted in their communities. This collaborative model has allowed HCN to become a nationwide network supporting community health centers across the country. Today, HCN serves and supports a network of Community Health Centers (CHCs) and safety-net provider organizations through key strategic services focused on improving access to quality health care for all and reducing disparities in minority and underserved communities. The organization drives innovation in technology and analytics, value-based care, clinical quality improvement, workforce and leadership development, and financial sustainability. As a 501c(3) nonprofit, HCN is guided by its founding vision of transforming healthcare and uplifting the communities they serve.
