Are you an LPN who enjoys building relationships, coordinating care, and helping residents successfully transition into a healthcare community? We're seeking an Admissions Coordinator LPN to lead the referral and admission process, ensuring a smooth experience for residents, families, hospitals, and referral partners.
This is a full-time opportunity.
Responsibilities
- Collaborate with nursing leadership and interdisciplinary teams to assess resident needs.
- Inquiry calls and tours for short term rehab and long term nursing care

Coordinate admission paperwork, authorizations, equipment, and services.

Conduct facility tours and help grow census through exceptional customer service.
Qualifications
- Valid Illinois LPN License
- Strong clinical assessment skills
- Previous Medicare admissions preferred
- Long-term care or healthcare experience preferred
- Ability to multitask
- Must have the desire to work with and serve seniors and their families
