We are seeking a passionate and culturally grounded AOD Counsellor / Case Manager (Identified) to join our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region.
This important role will support Aboriginal and Torres Strait Islander peoples throughout their recovery journey, helping strengthen engagement, access to services, cultural connection and long-term recovery outcomes. Working across pre-treatment, residential treatment and post-treatment pathways, you will support culturally safe and holistic care for individuals, families and communities.
About the Role
As the AOD Counsellor / Case Manager (Identified), you will provide culturally safe support, advocacy, case management and recovery-focused interventions for Aboriginal and Torres Strait Islander clients engaging with the Tokoko Integrated Recovery Pathways Program.
You will work alongside clients, families, Elders, community organisations and service providers to strengthen connections to culture, community and support networks. You will play a key role in reducing barriers to treatment, supporting transitions through the recovery journey and promoting culturally safe pathways into and beyond residential rehabilitation.
Key Responsibilities

Provide culturally safe engagement, support, case management and advocacy.
Support Aboriginal and Torres Strait Islander clients through pre-admission, treatment and post-treatment pathways.
Assist clients to develop and achieve their recovery and wellbeing goals.
Strengthen connections to family, culture, community and support networks.
Support access to health, housing, social, wellbeing and community services.
Build and maintain relationships with Aboriginal and Torres Strait Islander organisations, Elders and community stakeholders.
Advocate for the needs of Aboriginal and Torres Strait Islander clients and families.
Contribute to culturally safe service delivery and recovery planning.
Support continuity of care through effective referrals, stakeholder collaboration and community engagement.
Maintain accurate documentation, case notes and reporting requirements.

This role is based in Bundaberg and will involve travel across Bundaberg, Hervey Bay and surrounding communities.
What We're Looking For
To succeed in this role, you will bring:

Qualifications in Community Services, Human Services, Social Work, Counselling, Alcohol and Other Drugs, Mental Health or a related discipline, and/or relevant experience.
Experience working with Aboriginal and Torres Strait Islander peoples, families and communities.
Experience in case management, support work, counselling, advocacy or community engagement.
Understanding of Alcohol and Other Drug treatment, recovery and harm minimisation principles.
Strong understanding of culturally safe, trauma-informed and person-centred practice.
Ability to build trust and establish effective relationships with clients, families, Elders and communities.
Knowledge of local health, housing and community support systems.
Strong communication, advocacy and interpersonal skills.
Ability to work autonomously while contributing positively within a multidisciplinary team.
Current Australian Driver Licence.

Experience within the Alcohol and Other Drug sector will be highly regarded.
