We are seeking a passionate and experienced AOD Counsellor / Case Manager to join our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region.
This is an exciting opportunity to support people impacted by alcohol and other drug challenges through evidence-based counselling, case management and recovery-focused support. Working as part of a multidisciplinary team, you will help individuals strengthen treatment engagement, overcome barriers to recovery and build sustainable pathways towards long-term wellbeing.
About the Role
As the AOD Counsellor / Case Manager, you will provide therapeutic support, counselling and case management services to people experiencing alcohol and other drug challenges.
You will work closely with clients across all stages of their recovery journey, supporting treatment readiness, recovery planning and ongoing engagement before, during and after residential rehabilitation. Working within an integrated recovery pathways model, you will help people build the skills, supports and connections needed to achieve sustainable recovery outcomes.
Key Responsibilities

Provide high-quality engagement, assessment, counselling and recovery-focused support services.
Develop, implement and review individual recovery and treatment plans, including harm minimisation and psychoeducation.
Deliver evidence-based counselling interventions.
Support clients to identify and achieve their recovery goals.
Provide case management and coordinated support services.
Assist clients to access health, housing, wellbeing and community supports.
Support treatment readiness, recovery planning and relapse prevention activities.
Maintain continuity of care through referral, advocacy and stakeholder collaboration.
Maintain accurate client records, case notes and reporting requirements.
Participate in multidisciplinary case discussions and service planning activities.

This role is based in Bundaberg and will involve regular travel throughout the Wide Bay region.
What We're Looking For
To succeed in this role, you will bring:

Qualifications in Social Work, Counselling, Psychology, Community Services, Human Services or a related discipline, and/or relevant experience.
Demonstrated experience delivering counselling and/or case management services.
Knowledge of Alcohol and Other Drug treatment, recovery and harm minimisation principles.
Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
Experience supporting people with complex psychosocial needs.
Well-developed assessment, care planning and case management skills.
Strong interpersonal, communication and engagement skills.
Ability to build collaborative relationships with clients, families and service providers.
Strong organisational and documentation skills.
Current Australian Driver Licence.

Experience within Alcohol and Other Drug services will be highly regarded.
