Position Summary\:

City of Hope is seeking a dedicated Clinical Nurse to join our Case Management team, specializing in cancer treatment. In this role, you will play a crucial part in coordinating patient care along the continuum, advocating for patients, and ensuring their satisfaction while focusing on cancer care. This opportunity is for full-time (5 shifts per week) and 8-hour day shifts. Case Managers (full or part-time) are eligible for an additional up to $15.37/hr for weekend shift differentials.

As a successful candidate, you will\:

• Utilize the Case Management process to effectively communicate and coordinate patient care, ensuring a seamless experience for patients receiving cancer treatment.
• Serve as a patient advocate, actively promoting customer satisfaction by addressing patient concerns and facilitating their needs.
• Collaborate with healthcare teams to assess and plan patient care, ensuring timely access to necessary resources and services.
• Monitor patient progress and identify opportunities for care improvement, working to enhance the overall quality of care delivery.
• Utilize your BSN degree and clinical expertise to contribute to the overall success of the Case Management department.
