Overview

The Community Health Coordinator is responsible for providing health screenings at community events and through the mobile clinic. This position focuses on delivering essential healthcare services to underserved populations throughout Eastern Kentucky and Southern West Virginia.

The Community Health Coordinator plays a key role in promoting health education, preventing chronic disease, and improving access to healthcare services in rural and remote communities. Through community outreach and engagement efforts, this position supports the organization's mission to improve the health and well-being of the populations served.

Responsibilities

  • Assist with performing basic health screenings, including blood pressure, cholesterol, blood glucose, BMI, and other preventive health assessments.
  • Greet patients, explain screening procedures, and ensure patient comfort, privacy, and confidentiality.
  • Provide basic health education regarding screening results, preventive care measures, and healthy lifestyle choices.
  • Accurately document and enter patient information, screening results, and other relevant data into electronic medical record (EMR) systems.
  • Assist with the setup, operation, and breakdown of the mobile clinic, ensuring equipment and supplies are properly maintained, stored, and transported.
  • Collaborate with nurses, physicians, health educators, and other healthcare professionals to ensure high-quality patient care and appropriate follow-up services.
  • Represent ARH at health fairs, screening events, and community outreach activities, fostering positive relationships within the communities served.
  • Maintain high standards of patient care, safety, infection prevention, and hygiene during all patient interactions and procedures.
  • Travel throughout the service area, including rural and remote communities, to conduct health screenings and outreach activities.
  • Perform other duties as assigned.

Qualifications

Required Education

  • High School diploma or equivalent required.
  • Medical Assistant certification (CMA, RMA, or equivalent) required.
  • CPR/First Aid certification required or must be obtained within thirty (30) days of hire.

Required Experience

  • Minimum of one (1) year of experience as a Medical Assistant, preferably in a community health or outreach setting.
  • Experience performing health screenings, including blood pressure, glucose, and cholesterol screenings.
  • Familiarity with electronic medical record (EMR) systems.

Preferred Experience

  • Experience working with underserved populations or in rural healthcare settings preferred.
  • Experience in mobile health settings or mobile clinics preferred.
  • Knowledge of rural health issues and challenges, particularly within the Appalachian region, preferred.

Required Skills, Knowledge, and Abilities

  • Strong interpersonal skills with the ability to communicate effectively with patients from diverse backgrounds.
  • Ability to work independently and as part of a team.
  • Excellent organizational skills, attention to detail, and the ability to multitask.
  • Ability to handle sensitive patient information with confidentiality and professionalism.
  • Willingness and ability to travel to various locations, including rural and remote areas.
