Individual Support and Community Habilitation (ISCH) Caseworker

Employment Type: Full time/Part time

Work Setting: This position involves in person visits with individuals in their apartments, attending housing or benefit appointments in the community, and completing some tasks remotely.

Position Summary

The ISCH Caseworker supports individuals living in the community through the waiver program by helping them grow more independent and confident in their daily lives. This role requires in person visits, assisting with paperwork, creativity in engaging a diverse population of individuals served, and flexibility to adapt to changing goals and needs. ISCH Caseworkers assist with housing recertifications, health insurance and benefit maintenance, and daily supports such as transportation, budgeting, and social engagement.
