Enhanced Care Management (ECM) Program Manager
Summary
Program Manager builds and runs a new child/family healthcare initiative from scratch, ensuring Medi-Cal compliance and whole-person care coordination across Alameda County.
Under general direction, the Program Manager leads the development and implementation of First 5 Alameda County's new Enhanced Care Management (ECM) program for young children and families with complex medical and social needs. This is a unique opportunity to build a program from the ground up that will improve access to coordinated, whole-person care for children and families across Alameda County. The Program Manager oversees program implementation, supervises staff, and ensures services align with CalAIM requirements, quality standards, and regulatory expectations. In partnership with clinical and community partners, the role develops workflows, protocols, and systems that support effective ECM service delivery while advancing child-centered care and birth equity.
- Oversees delivery of ECM services and ensures alignment with CalAIM requirements.
- Supervises and supports ECM staff to ensure high-quality, coordinated service delivery.
- Supports the development and review of comprehensive care plans to ensure a coordinated whole-person approach for Medi-Cal members
- Develops and refines operational workflows and processes to support effective ECM implementation and program operations.
- Coordinates with Managed Care Plan partners to support program alignment and ensure compliance with contractual, quality, and promising practices standards.
- Ensures accurate and timely staff documentation to support billing requirements and necessary reports.
- Assesses team performance and communicates findings to leadership; implements changes to improve efficiency and service quality; maximizes effectiveness and ensures alignment with department’s mission
- Recommends and implements goals and objectives, policies and procedures and ensures the program is staffed with qualified employees by supporting hiring, training, performance management, and staff development
- Oversees daily program operations by setting expectations, communicating goals, and monitoring work to ensure quality, timeliness, and compliance with policies
- Develops and implements program plans and resources, and provides training to ensure staff and partners understand their roles and responsibilities
