Licensed Practical Nurse

We are seeking an LPN to support as-needed patient visits for our INR testing, Sleep (PAP setup), and AffloVest therapy programs. This is a paid-per-completed-service independent contractor role (not hourly), with mileage reimbursement. You will provide in-home or mobile patient support across these three service lines, only as visits are needed.

What You'll Do:

• Perform in-home INR point-of-care testing and report results to the ordering provider or clinical team
  
• Complete PAP (CPAP/BiPAP) setups, including mask fitting and equipment education
  
• Complete AffloVest patient setups and education visits
  
• Make sure patients understand proper use, compliance, and therapy goals
  
• Reinforce provider instructions
  
• Troubleshoot basic fit, comfort, and equipment usage issues
  
• Document visit completion, results, and patient status
  
• Coordinate with clinical teams as needed
  

Pay Structure:

• Paid per completed service / visit
  
• Mileage reimbursement
  
• 1099 independent contractor (PRN)
  
• Flexible scheduling: accept jobs as available
  

Requirements:

• Active LPN license (state of assignment)
  
• Experience in home health, DME, sleep, or respiratory care preferred
  
• INR / point-of-care testing experience a plus
  
• Strong patient education skills
  
• Reliable transportation
  
• Comfortable with mobile / in-home visits
