Responsibilities:
- Hospice visits - daytime visits
- Provide accurate and timely documentation
- Assess and provide patient and family/caregiver education
- Perform other duties as assigned by the registered nurse
- Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.
Qualifications:
- Demonstrated compassion and commitment to patient-centered care
- Active LVN license in the state of California
- Valid California driver’s license and reliable transportation
- Minimum 1 year of hospice nursing experience; strong understanding of hospice philosophy and end-of-life care
- Prior experience in nursing or related medical fields
- HomecareHomebase/Pointcare EMR: 1 year (Preferred)
Job Types: Part-time

- Weekdays
Service Coverage:
- Orange County
Compensation: $30.00 - $35.00 per hour.
