Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a Full-Time Licensed Physical Therapist Assistant (LPTA/PTA) to join our dedicated therapy department.

Located on a beautiful, serene, wooded campus in Ohio, we are a family-owned and operated skilled nursing facility committed to providing exceptional, resident-centered rehabilitation in a warm, welcoming, and supportive environment.

Position Summary

Under the direction and supervision of a licensed Physical Therapist, the Physical Therapist Assistant provides skilled physical therapy treatments to help residents improve mobility, strength, balance, and overall functional independence.

Key Responsibilities

• Administer physical therapy treatments according to established plans of care.

• Assist residents with therapeutic exercises, gait training, and functional mobility.

• Monitor and document resident progress and responses to treatment.

• Collaborate with Physical Therapists and the interdisciplinary care team.

• Promote resident safety, dignity, and individualized care.

• Follow facility policies, procedures, and regulatory requirements.
