Job Overview

We are looking for an experienced Medical Concierge with a nursing background to coordinate residents' healthcare needs, personalised care plans and medical services. This role requires strong clinical knowledge and independent working capabilities to deliver seamless, resident-centred care.

Key Responsibilities

• Develop and review individualised care plans to support residents' healthcare and wellbeing needs.

• Monitor residents' health conditions, vital signs and care outcomes, escalating concerns when necessary.

• Coordinate medical appointments, specialist consultations, hospital visits and follow-up care.

• Liaise with residents, families and healthcare providers to ensure effective care coordination and timely communication.

• Support resident admissions, health screenings, wellness programmes and healthcare transitions.

• Maintain accurate medical records and ensure compliance with healthcare standards, incident management and quality improvement procedures.

Requirements

• Degree in Nursing with at least 3 years of relevant healthcare or care coordination experience.

• Candidates with experience as a Nurse Manager, Senior Nurse or Advanced Practice Nurse (APN) are encouraged to apply.

• Strong clinical knowledge and experience in care coordination, case management or resident care.

• Excellent communication, interpersonal and stakeholder management skills.

• Independent, organised and comfortable engaging elderly residents from diverse backgrounds, including Mandarin-speaking residents.
