Summary/Objective |
The Optometrist is expected to be familiar with the philosophy, goals and objectives of the PCMH model, and work collaboratively with other members of the patient care team to achieve maximal health and vision outcomes for NSHC patients. In this role, the Optometrist will examine NorthShore Health Center patients’ eyes to determine the nature and degree of vision problems or eye disease, and prescribe corrective lenses or procedures according to the scope of their licensing. The Optometrist must meet and maintain credentialing guidelines of NorthShore Health Centers, and for professional licensing with the State of Indiana. |
