PACE REHABILITATION MANAGER

The PACE Rehabilitation Manager provides leadership, direction, and oversight for rehabilitation and recreational therapy services across all PACE sites. This role supervises PTs, OTs, Rehab Aides, RTs, activity coordinators, activity assistants, and activity instructors, ensuring scheduling, training, performance management, regulatory compliance, participant satisfaction, and continuous quality improvement. The PACE Rehabilitation Manager leads coordination of PPAC activities and participant survey administration, activity instructor contracting/management, emergency preparedness training, and QI initiatives specific to rehabilitation services.

ESSENTIAL JOB FUNCTIONS:

  • Provide direct supervision, training, coaching, and performance evaluation for PTs, OTs, Rehab Aides, RTs/activity coordinators/activity assistants, and activity instructors.
  • Ensure effective hiring, orientation, ongoing training, scheduling, coverage, and staffing across all PACE sites for rehabilitation services.
  • Delegate responsibilities appropriately according to licensure, credentials, education, and experience.
  • Lead PPAC coordination and oversee administration of all participant surveys.
  • Lead activity instructor/vendor contracting, onboarding, scheduling, performance oversight, and vendor relationship management.
  • Lead trainings for emergency preparedness (center and community protocols) for all staff and ensure training documentation and drills are completed.
  • Develop, review, and maintain policies and procedures for rehabilitation service delivery, clinic operations, and community based rehabilitation activities.
  • Lead QI initiatives related to rehabilitation services, including facilitating the Fall Subcommittee, tracking QI metrics, and implementing corrective actions in collaboration with the Medical Director and interdisciplinary team.
  • Monitor encounter, assessment, care plan, and center attendance metrics; analyze data to identify opportunities to improve care quality and participant experience.
  • Implement strategies to meet or exceed national and state participant satisfaction benchmarks for rehabilitation and recreational therapy services.
  • Coordinate closely with PCPs, IDT members, and community providers to develop and implement individualized care and therapy plans.
  • Oversee rehabilitation coordination with SNFs, RCFEs, home health, hospice, and other external providers to ensure continuity of care.
  • Coordinate and schedule home health rehabilitation visits.
  • Oversee the operational aspects of rehabilitation equipment, supplies, and space utilization across sites.
  • Resolve grievances related to rehabilitation issues in collaboration with the Center Manager.
  • Prepare for and participate in CMS, DHCS, and other regulatory audits and implement corrective actions as needed.
  • Ensure accurate, cohesive, and timely documentation in participant medical records for all rehabilitation services.
  • Oversee the quality of care and safety of PACE participants, both at the PACE center and in the community.
  • Ensure compliance with all PACE regulatory requirements and internal policies.
  • Travel to other PACE sites as needed for supervision, training, audits, or to address participant/staff issues.
  • Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
  • Perform other job duties as required by manager/supervisor.
