Center of Illinois Home Healthcare, Inc. is seeking a compassionate and motivated Per Diem Physical Therapist to join our Medicare-certified, ACHC-accredited home health agency. This position offers the flexibility to create your own schedule while providing high-quality physical therapy services to patients in their homes.

As a member of our interdisciplinary team, you will evaluate, develop, and implement individualized treatment plans that promote independence, improve mobility, and enhance patients' quality of life.

Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care.
• Assess patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on therapeutic exercises, mobility, fall prevention, and home safety.
• Complete timely and accurate documentation in accordance with Medicare, CMS, and agency guidelines.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary team.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies, state regulations, and accreditation standards.
