Program Support Specialist/Service Facilitator (3745)
Job Summary:
The Program Support Specialist assists with the organization, administration, and coordination of the operations of the. The Program Support Specialist/Service Facilitator is a hybrid administrative and clinical position in a State of Wisconsin certified Comprehensive Community Services (CCS WI DHS 36) program and participates as member of an interdisciplinary treatment team to assist clients and related team staff. Primary duties include secondary service facilitation, data entry and transfer, fax and CCS Rx management, providing in-person and telephone support to clients and visitors, and serving as an office administrative resource specialist to the rest of the team.

Duties/Responsibilities:
A. Administrative (High responsibility: 50% of time)
- Clinical note and data transfer from JMHC electronic health record to the Dane County CCS portal.
- Billing submission to Dane county CCS portal
- Fax management of lab reports, records requests, medication refill requests, CCS prescription submissions, and all other miscellaneous duties.
- Provide in-person and telephone support to clients and visitors.
- Serve as an office administrative resource specialist to the clinical teams.
- Act as a front desk liaison for the CCS GROW service facilitation and medical teams.
- Support with record keeping and completion of key documents for staff. Including but not limited to scanning and uploading documents into EHR systems and sending documents into Dane County
B. Clinical (High responsibility: 40% of time)
- Serve as a secondary or “back up” CCS service facilitator supporting both the CCS GROW service facilitation and medical teams
- Assist the CCS GROW medical team with documentation completion, scheduling, clinical notes and data entry.
C. General Program and Agency Responsibilities (Low Responsibility: 10% of time)
- Attend and participate in team meetings and trainings.
- Coordinate activities and/or meetings with other Program Support Specialists.
- Work actively and positively to maintain good working relationships between other JMHC programs, vendors and other community members.
- Work to improve own cultural sensitivity by participating in the development and implementation of program strategies to continue to improve the cultural sensitivity of the program’s services.
