Concordia Visiting Nurses is seeking a dedicated and adaptable Triage Registered Nurse to join our team! As a Triage Registered Nurse you’ll play a key role in coordinating and overseeing patient care. You’ll have the opportunity to work in a supportive and collaborative environment, making a meaningful impact on patient outcomes and the well-being of our community. This is a 32 hour per month part-time/casual position. We value flexibility and work-life balance, so the role allows for some variation in hours to meet both the needs of our patients and your personal schedule
QUALIFICATIONS for the Triage Registered Nurse position:

Current Registered Nurse (RN) license in the state of Pennsylvania
Minimum of [2] years of current experience as a professional nurse in a medical/surgical environment and [1] year home health experience recommended.
Possesses strong assessment, problem solving and decision-making skills
Expertise in pain management, symptom control, crisis intervention, and family support

Skilled in critical thinking, communication, negotiation, collaboration and customer service.
Excellent organizational skills with attention to detail
Outstanding communication skills (both written and verbal); must communicate in a warm, professional manner

RESPONSIBILITIES for the Triage Registered Nurse position:

The Triage Nurse is responsible to utilize the company telephone and laptop computer to access the patient record during after hours. The clinician will also have a current on-call schedule to identify what clinicians are on-call, and what assignments they have received already for the evening hours.
Via Telecommunication method, the clinician should assess the patient signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in the patient physical or emotional condition to the physician.
Clinician should have ability to triage patient via telecommunication and assess what further care is indicated by CVN staff or ability to talk to patient and communicate what care needs provided over the phone.
The clinician should possess the ability to delegate visit responsibility to the clinicians’ on-call. Or when indicated, to notify patient to seek emergent care via hospital emergency room.
Instructs and counsels the patient and caregiver regarding care requirements of the patient in the home.
The Triage Nurse will enter a note into the Case Communication note as an On-Call Note to document the communication between the Triage Nurse and the patient. The documentation will include the clinician’s instructions to the patient and the visiting clinician and/or the patient primary field clinician. The Triage Nurse will also document the clinicians name and time directive was given.
The Triage Nurse will be responsible to complete the intake process for after hour referral calls.
