As we continue to expand our post-acute care services, we are seeking a compassionate, dependable, and motivated Physician Assistant (PA-C) or Nurse Practitioner (NP) to join our team as a SNFist, providing medical care to patients in skilled nursing facilities.

Position Summary

The SNFist PA-C or NP will provide comprehensive medical management to patients residing in Skilled Nursing Facilities (SNFs). This position is ideal for a provider who is comfortable managing medically complex adult and geriatric patients and working collaboratively with physicians, nursing staff, rehabilitation teams, specialists, and facility leadership.

The provider will be responsible for evaluating patients, managing acute and chronic conditions, completing follow-up visits, coordinating care, and supporting safe transitions throughout the patient's post-acute stay.
