Responsibilities:
- Provide administrative and operational support for the department, including front counter management and related duties.
- Conduct intake interviews, perform financial assessments, and provide appropriate assistance to patients and families.
- Facilitate referrals and support discharge planning activities for selected areas of care coordination.
- Assist Medical Social Workers with case management activities, including translation support, patient escort services, and home visit assistance.
- Participate in process improvement and quality improvement initiatives.
- Collaborate with internal departments and external community partners to enhance patient care and service delivery.
- Support departmental programmes, projects, and initiatives.
Requirements:
- Diploma in Social Work from a recognized institution.
- Good interpersonal and communication skills with the ability to work effectively with patients, families, and multidisciplinary teams.
- Strong problem-solving and organizational skills.
- Proficient in Microsoft Office applications and general IT systems.
- Ability to manage administrative responsibilities while supporting client-facing activities.
- Able to work collaboratively in a fast-paced healthcare environment.
