Social Work Coordinator

Responsibilities:

• Provide administrative and operational support for the department, including front counter management and related duties.
• Conduct intake interviews, perform financial assessments, and assist patients and families in accessing available financial assistance schemes.
• Support discharge planning and care coordination through referrals and collaboration with relevant stakeholders.
• Assist Medical Social Workers with case management activities, including translation support, patient escort services, and home visit assistance.
• Participate in process improvement and quality improvement initiatives.
• Collaborate with internal departments and external agencies to enhance patient care and overall patient experience.
• Support departmental programmes, projects, and community initiatives.

Requirements:

• Diploma in Social Work or a related discipline.
• Good interpersonal, communication, and problem-solving skills.
• Ability to work effectively with patients, families, healthcare professionals, and community partners.
• Proficient in Microsoft Office applications and general IT systems.
• Ability to manage multiple tasks in a fast-paced healthcare environment.
• Experience in social work, healthcare, community care, or related settings is an added advantage.
