Social Work Coordinator – Hospital | Up to $3,000 #HHLT

💰 $2,500–$3,000/month

💼 Responsibilities
• Support the daily administrative and operational functions of the Medical Social Work department
• Manage counter duties and attend to patients and families
• Conduct intake interviews and financial assessments
• Provide appropriate financial assistance and support to patients
• Assist with referrals and discharge planning
• Support Medical Social Workers with case management activities
• Provide translation, patient escort and home visit support when required
• Coordinate with hospital departments and external organisations to facilitate patient care
• Participate in process and quality improvement initiatives
• Support departmental programmes and projects

📝 Requirements
• Min. diploma in Social Work required
• Good interpersonal and problem-solving skills
• Comfortable interacting directly with patients and families
• Good IT knowledge and computer skills
• Able to work closely with Medical Social Workers and multidisciplinary teams
