Speech-Language Pathologist (SLP) – Hybrid/Remote

LifeCare Home Health & Hospice Family is seeking a Speech-Language Pathologist (SLP) to join our home health team serving Weatherford, TX, and surrounding areas. This position offers a combination of virtual treatments and in-person visits, giving you flexibility while ensuring patients receive the quality care they deserve.

The ideal candidate will live in or near Lubbock, TX, to accommodate required in-person visits. While routine treatments may be completed virtually when clinically appropriate, this is not a fully remote position.

What You’ll Do:

• Evaluate patients and develop individualized speech therapy treatment plans.
• Complete required in-person re-evaluations every 30 days within the designated one-week window.
• Provide routine speech therapy treatments virtually 1–2 times per week when clinically appropriate.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on treatment techniques and strategies.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
• Utilize Nestmed AI Scribe, an AI-powered documentation tool designed to help streamline clinical notes and reduce administrative time.
