Hiring range: $60,465.60 - $83,283.20
The actual compensation for this position will be determined based on experience and other factors permitted by law.

Primary Job Responsibilities:

  • Conduct timely, comprehensive assessments of patients presenting with behavioral health crises in the ED for adults, adolescents, and children
  • Develop and implement individualized crisis intervention and stabilization plans.
  • Provide clinical recommendations regarding level of care, including inpatient admission, outpatient services, or community-based alternatives.
  • Coordinate care transitions by facilitating appropriate referrals to inpatient, outpatient, and community-based services.
  • Collaborate with multidisciplinary teams (physicians, nurses, social workers, and external providers) to ensure cohesive care planning.
  • Assist with discharge planning, including identifying barriers to care and arranging follow-up services.
  • Maintain knowledge of community resources and actively connect patients to appropriate supports (e.g., mental health providers, substance use treatment, housing resources).
  • Monitor high-risk or high-utilization patients and participate in strategies to improve continuity and reduce recidivism.
  • Complete accurate, timely, and thorough clinical documentation in accordance with regulatory and organizational standards.
  • Ensure documentation supports clinical decision-making, care coordination, and continuity of care.
  • Maintain compliance with hospital policies, state regulations, and accreditation standards.
  • Participate in quality improvement initiatives aimed at enhancing patient outcomes, care transitions, and ED throughput.
  • Contribute to development of protocols that integrate crisis services with ongoing care management.
  • Track and analyze trends related to behavioral health presentations, dispositions, and service gaps.

Supervisory Responsibilities:

  • Provide clinical guidance, consultation, and supervision to Crisis Evaluators and/or behavioral health staff.
  • Assist with workflow coordination and prioritization of patient assessments within the ED.
  • Support onboarding and training of new staff, emphasizing both crisis evaluation and care coordination practices.
  • Serve as a clinical resource to ED staff regarding behavioral health and crisis management.

Minimum Qualifications:

Education, Training, and Experience:

  • Master's degree required within Twelve (12) months from the date of hire or transfer.

Certifications/Licensure:

Preferred Credential(s):

One or more of the following:

  • Counselor credentialed from the New York Board of Mental Health Practitioners obtained within 18 Months (1 1/2 year) of hire date or job transfer date.
  • Psychologist credentialed from the New York Board of Psychology obtained within 18 Months (1 1/2 year) of hire date or job transfer date.
  • Social Worker credentialed from the New York Board of Social Work obtained within 18 Months (1 1/2 year) of hire date or job transfer date.
