Support Practitioner (Derwen Care)

We have exciting opportunity for a part-time Support Practitioner (weekends) to join our Derwen Care team! For this role you will be required to support our clients.

Job Specification:

• Deliver person centered care putting the client at the heart of everything and helping them when needed to make their lives easier and improve their quality of life
• Take an active role in supporting clients to develop their personal care and daily life skills.
• Active involvement, contribution and participation in team delivery ensuring all clients are empowered to be as independent as possible.
• Support clients to attend external appointments accessing a variety of different transport methods
• Administration of client medication and support of client self-administration of medication in line with college policy and procedures
• Take on the role of keyworker for identified clients acting as a point of contact for parents/carers.
• Liaison with families, careers, support network and key support services to ensure relevant, effective communication
• Encourage and promote positive social integration and inclusion within client peer groups where appropriate internally and externally
