This is a full time, direct care position. The primary responsibilities of the Extender are to:
- Perform general outreach, engagement, and follow-up with members.
- Spanish language proficiency preferred.
- Coordinate services/appointments.
- Engage clients in health promotion activities.
- Share information with Care Manager on the member's circumstances.
- Make and track referrals.
- Participate in case conferences.
- Support the care manager in assessing and addressing unmet health-related resource needs.
