PATHS, LLC is seeking a dependable and patient-focused Field Representative to support individuals within the Delaware area. This role is ideal for candidates who enjoy working independently, traveling within their assigned territory, and interacting directly with patients in community and home-based settings.
As a Field Representative, you will assist patients with Medical Assistance applications, gather required information, and serve as a trusted liaison between patients and healthcare services.

Key Responsibilities
- Assist patients with applying for Medical Assistance (Medicaid)
- Travel within a 100–200 mile radius of the assigned territory (mileage reimbursed)
- Enter client facilities and communicate effectively to obtain required patient information
- Accurately document patient data and complete assigned tasks in a timely manner
- Transport supplies to and from client facilities as needed
- Maintain compliance with confidentiality and healthcare regulations
- Perform other related duties as assigned
