Veteran Support Team Lead - BH Grants

The Veteran Peer Support Specialist Team Leader is a veteran responsible for leading and overseeing a team of veteran peer support specialists tasked with providing compassionate, recovery-oriented support services to individuals experiencing mental health and/or substance use concerns, based on their own lived experience. The Veteran Peer Support Specialist Team Leader will assist with providing support services and with linking the individual to resources that will address both their behavioral health needs, as well as their Social Determinants of Health (SDOH) needs including but not limited to, housing and employment and medication management.

Essential Functions

• Use own experience of recovery to help others.
• Assist individuals in identifying and utilizing resources in the community for additional behavioral health, recovery, and SDOH support.
• Maintain accurate documentation of patient interactions and progress and maintain confidentiality of all personal information.
• Develop and maintain knowledge of local, state, and federal services that may provide resources to patients and families.
• Encourage self-advocacy among individuals to help them achieve their goals.
• Participate in ongoing training and professional development opportunities.
• Assist patients in identifying and achieving their personal behavioral health goals, including recovery.
• Schedule, assign work to and evaluate the work performance of employees supervised.
