Reporting Relationships: Case Manager reports to Assistant Program Director.

Principal Duties and Responsibilities:

Under the general supervision of the Assistant Program Director, Case Managers are expected to perform the following principal duties and responsibilities.

  • Conduct intake, in-depth assessments and interviews to determine eligibility for the program prior to program entry.
  • Use a Person-Centered approach to help tenants set goals, identify interests, and address concerns.
  • Get to know tenants and their psychosocial history in order to provide strategic interventions and services.
  • Develop and update support plans, case notes and other documents related to a tenants file and in accordance with funder requirements.
  • Coordinate care and supports for tenants with the guidance of Assistant Program Director and the social services team.
  • Support tenants in exploring and pursuing employment, education, volunteering, and other activities.
  • Provide ongoing life skills development including problem solving and skill development to aid in the tenants' process of improving independence and quality of life.
  • Provide specialized and targeted interventions when tenants are at risk of eviction.
  • Advocate and connect tenants to medical, mental health, and other supportive counseling services.
  • Provide outreach in order to engage tenants who do not follow through with appointments and case management.
  • Regularly coordinate care with all collateral contacts, including but not limited to family, providers, ACS and APS, DOE, etc.
  • Maintain a strengths-based perspective to engage all members of the family in order to help improve family functioning.
  • Link tenants with vital community resources including medical, mental health, substance abuse treatment and support groups.
  • Engage with community partners to provide services onsite and offsite. Conduct visits in the tenant’s unit to ensure compliance with lease and program rules.
  • Attends all internal trainings, development conferences and other trainings scheduled by funding agency.
  • Enlist the support of property management as appropriate for wellness checks.
  • Effectively guide tenants through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, HRA, SNAP and other services as needed.
  • Ensures compliance with organizational standards of confidentiality in handling of records and maintains accurate and up to date tenant files.
  • Participate in program events and conduct weekly group activities.
  • Track and report services/outcomes and provide data updates on tenants according to organizational and funder requirements.
  • Performs other related duties as may be requested by immediate supervisor and in his/her absence the Program Director.
